Username: YOUR EMAIL ADDRESS

Password: YOUR PHONE NUMBER (NO SPACES OR DASHES)

After logging in for the first time, you can change your password by doing the following:
1. Click on the down arrow in the upper right hand corner of the web page
2. Select “My Profile”
3. Insert new password in field marked “Change Password”
4. Click Update

In your portal you can:
Access your records
View account history
Complete pending patient forms
Pay your balance
View upcoming appointments
